HOW TO REQUEST A REIKI TREATMENT SESSION WITH ME

If you would like to schedule a reiki treatment session with me, please take time to complete and submit the form below. I will make every effort to accommodate your desired appointment request. Unfortunately however, I am currently very limited on the time that I have available for offering reiki treatments.

After submitting your request, I will contact you to discuss my availability and we can work out the scheduling details. If you have any questions about Reiki or reiki treatment sessions, feel free to contact me via email at: reikiwithjotaro@gmail.com. 

REIKI TREATMENT REQUEST

After I receive your treatment request, I will contact you to discuss the details of the treatment session and set-up the date and time for your appointment. What is your preferred method of communication?

I offer a few different treatment options to help reduce the cost if you desire/need multiple treatment sessions. Please select the level of frequency that you wish the sessions to take place. Please note, chronic conditions may require multiple treatment sessions to achieve results.

Please take a moment to tell me about your reasons for seeking a reiki treatment session. List any reasons such as the issue you wish to address during the session and also if you are currently undergoing any treatment from other healing modalities or doctor care.

Payment Of Reiki Treatment Session Fees -

Fees for Reiki treatment sessions are as follows:

​

Single Treatment Session - $75

Package Of Three Treatment Sessions - $200 ($25 Savings)

Package Of Five Treatment Sessions - $300 ($75 Savings)

Treatment sessions can be paid either in advance via credit/debit card or at the time of the appointment. I accept payments using cash, personal check and credit/debit cards. Credit/debit card payments are processed by Square. How do you wish to pay for your reiki treatment session? ​

Reiki Treatment Session Cancellation Policy -

 

I fully understand that life is unpredictable and things sometimes come up forcing a change to our plans. If you have a scheduled treatment session with me, I would like to request that you contact me with as much notice as possible before your scheduled appointment time. This will allow for us to either reschedule the appointment or arrange to refund any fees that might have been paid.

If there is no request for rescheduling or no notice of cancellation for your appointment is given and you fail to show at the scheduled appointment time, I reserve the right to charge you a 'No Show' fee of $25 to compensate for the time not used.

BEFORE YOU GO

Before your appointment, please take a moment to print and complete the 'REIKI WITH JOTARO CLIENT INTAKE FORM' to the right. If you are not able to print it out before your appointment, this form will be available to fill out before your scheduled treatment session.

 

Also, look over the 'CLIENT BILL OF RIGHTS' this form is required by the State of Minnesota to be made available to those who come to me for reiki treatment sessions. If you desire, you can download this document for your own records
